The Rocky Mountain Chapter

It takes a village to transition complex patients safely and efficiently throughout the care continuum. More and more, transitions of care are tied to reimbursement and quality indicators. Understanding the role and importance of Long-term Acute Care Hospitals (LTACH) in that continuum can support improved patient outcomes, reduce costs, and reduce re-admissions. Our panel of LTACH CEOs will discuss the importance of understanding all levels of care, specifically, how to use LTACHs to decrease length of stay and decrease hospital re-admissions. They will discuss which patients are most appropriate and eligible for LTACH care.  Additionally, the panel will discuss how LTACHs fit into the healthcare continuum as this can often be a confusing level of care.

 

Objectives

 

At the conclusion of the presentation, the participant will be able to:

 

1.       Understand the various levels of care at Long-Term Acute Care Facilities (LTACH)

2.       Describe how to use LTACH’s to decrease length of stay and hospital readmissions

3.       Differentiate which of their patients are most appropriate and eligible for LTACH care

Deborah Dale, CEO

Select Specialty (Denver and Denver South)

Deborah has worked in healthcare management for over 30 years in a variety of settings such as hospitals, physician practice, Physical Rehabilitation, Surgery and LTACH.  She relocated to Denver to take the CEO position at a LTACH.  Deborah has a Bachelor of Science in Occupational Therapy and a Masters in Healthcare Administration.

 

James Elton, CEO

Select Specialty (Colorado Springs)

James has been an LTACH CEO for 2 years, and Administrator for Skilled Nursing for 8 years.  He has a BS in Business from BYU- Hawaii, and a Masters in Health Administration from Weber State in Utah. I like long walks on the beach, basketball, and painting.

 

Nicole Reinhardt, CEO

Kindred East

Nicole has been an RN for 15 years, specializing in critical care nursing. 3 years of Nicole’s nursing experience was spent as a home care case manager overseeing 25-30 patients at a time. Nicole became the CNO of an LTACH in 2009 and then the CEO in 2011. Nicole brings a wealth of knowledge to the administrative team with her nursing background and various areas of experience.

April 2013 Donations by the Chapter

During our April Inservice a member was selected to draw from a list of local charitable non profit agencies for donation. We plan to do this at each inservice in 2013.

Bi Monthly Meetings

CMSA Colorado meetings are the 3rd Tuesday of every other month and rotated to different locations. Meetings begin promptly at 5:30pm with networking and greetings. The presentation begins at 6:00pm and concludes by 7:30pm. CCM's are provided. We encourage all case managers, nurses, social workers, and healthcare professionals and affiliates to attend these meetings. The cost for members is $5 and non-members is $15-this cost includes the cost of dinner and CEU.

  • Create a secure user account to manage your Continuing Professional Development and Healthcare Professions Profile.

Continuing Professional Development Functions:

  • Attest to a military exemption or deem status.
  • Access the Professional Practice Survey.
  • Draft, update and submit your Learning Plan.

Healthcare Professions Profiling Program Functions:

  • Licensees will be able to complete their Healthcare Profession Profile after July 1, 2011.
  • Complete your Healthcare Profession Profile prior to renewing your license, certification or registration.
