Click here for the information about the HBU stakeholder conference. I would strongly encourage attendance as there seems to be real momentum in making changes to the Medicaid back-up programs. At the initial meeting there were very few providers. I would love to see the room full of providers that work with the programs to help make decisions. Keep in mind this program is not just for ventilator dependent patients, but any patient who is Medicaid pending, Medicaid eligible, or too acute for discharge for any reason.

Primary duties of this position include, but are not limited to: Serve as a member of the PACE interdisciplinary team. Demonstrate assessment skills, effective verbal/written communication skills, the ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials. Knowledge of community resources available to seniors and their families. Skilled in identifying and recommending problem resolution. Acts as liaison with participant and caregivers regarding orientation to and ongoing relations with interdisciplinary team, day center, and other Rocky Mountain PACE staff, including volunteers. Presents requests to interdisciplinary team for and coordinates admission/discharge to contracted facilities for temporary respites and permanent placement. Experience with discharge planning from hospital or skilled nursing facility settings a plus. Provides ongoing support, counsel, and education to participants and family regarding a variety of issues, including but not limited to the aging process, dementia, grief and loss, end of life, disease processes, difficult family dynamics, and changing roles. Will assist the Intake/Enrollment Case Manager with completion and review of participant financial eligibility applications.
The physical demands of this job include, but are not limited to: the ability to lift/carry pushing, bending, kneeling and reaching at arm's length for prolonged periods of time; working under stressful conditions.
Must have excellent documentation, typing, computer, multi-tasking and time management skills. Candidate must have a current driver's license in good standing, as well as reliable transportation. Will be responsible for maintaining case records on each client served, as well as communicating orally and in writing with clients and family members.
Salary Range: $45,000- $48,000 DOE.

Under the general supervision of the Manager, nurses in the Case Manager role provide clinically-based case management to support the delivery of effective and efficient patient care. Paces cases from physiological and economic perspectives. Has overall accountability for the utilization management and transition management for patients within the assigned caseload. Partners with Social Workers and collaborates with other health care team members to identify appropriate utilization of resources and to ensure reimbursement. Utilizes criteria to confirm medical necessity for admission and continued stay. With the patient, family and health care team, creates a discharge plan appropriate to the patient's needs and resources.
Candidates must be able to demonstrate complex case management experiences.
Case Management Certification Preferred.
Prefer candidates with a psychatric background.
