Yoh Health Care is looking for a RN Case Manager in the Denver area. This opportunity is a full-time, direct-hire position. The Case Manager is responsible for disability case management by providing medical knowledge and working with all the providers. This is a telephonic case management position. Candidate must be a RN with a minimum of 2-5 years medical experience in occupational health, workers' compensation or disability case management. The candidate must have solid system and communication skills. Strong clinical background required. The ideal candidate possesses the following:
- Active Colorado RN License
- Minimum of 2-3 years of case management
- Experience with case management of disability, FMLA, and Workers Compensation
- Must have an Associate Degree; BSN highly preferred
- Certified Case Manager Certification is a plus

Colorado Rehabilitation
Hospital (NCRH) has been
rated in the Top 10% for patient outcomes in each of the past four years. We are looking for an RN to join our team of
passionate caregivers.
NCRH is a 40 bed, inpatient rehabilitation hospital, located
in Johnstown Colorado
(near Loveland, Fort Collins
and Greeley). Our hospital, designed solely for the
provision of specialized rehabilitative care, is the only freestanding acute
rehabilitation hospital serving northern Colorado.
The facility's unique design fosters a comfortable and responsive environment
for our patients. Our hospital provides rehabilitative services for patients
with functional deficits as a result of debilitating illnesses, or injuries
such as stroke or trauma.
The case manager functions as the coach of the
treatment team consisting of the patient and family, physician(s) and clinical
staff. The case manager coaches the team during the delivery of care, and
provides appropriate case follow up. The case manager participates, as
appropriate, in patient rounds, medical staff seminars, nursing staff
conferences, and in conferences with individual physicians, nurses, and other
personnel concerned with the care of a patient and the patient's family.
REQUIREMENTS:
- Current
Clinical License in the State of Colorado
or eligible to practice in their healthcare field of expertise.
- Eligible
to sit for the national CCM exam.
- Minimum
of three (3) years case management experience preferred.

One year of experience in the care of patients in
rehabilitation hospital setting preferred.

Working knowledge of MS Office software preferred.
- Not
listed under the CMS excluded provider provisions required.
