The facility's unique design fosters a comfortable and responsive environment for our patients. Our hospital provides rehabilitative services for patients with functional deficits as a result of debilitating illnesses, or injuries such as stroke or trauma.   The case manager functions as the coach of the treatment team consisting of the patient and family, physician(s) and clinical staff. The case manager coaches the team during the delivery of care, and provides appropriate case follow up. The case manager participates, as appropriate, in patient rounds, medical staff seminars, nursing staff conferences, and in conferences with individual physicians, nurses, and other personnel concerned with the care of a patient and the patient's family.   REQUIREMENTS:  Current Clinical License in the State of Colorado or eligible to practice in their healthcare field of expertise. Eligible to sit for the national CCM exam. Minimum of three (3) years case management experience preferred. One year of experience in the care of patients in rehabilitation hospital setting preferred. Working knowledge of MS Office software preferred.
